Mentor Way Nursing & Rehabilitation Center, Inc. delivers this by providing an interdisciplinary approach to your loved one’s care by a team of licensed medical professionals.

We also want our residents to enjoy the comforts in our home-like environment, which maximizes their freedom and safety within our secure setting. As a long term care facility and rehabilitation center, we can provide a level of care individualized for each resident’s specific needs, ranging from minimal assistance in activities of daily living to the medically complex resident.

For Admission; referring physicians, discharge planners and family members should contact the Admissions Coordinator at (440) 255-9309 to answer any questions, schedule a tour of the facility and begin the process of admission.

Admission / Social Services
The Admission / Social Service Department recognizes the difficulties associated with nursing home placement. Throughout the admission process, we are committed to educating family members and residents about our facility and the procedures associated with placement and discharge planning.

During this transition period we are conscious of the needs of both the family and resident, and have a fully licensed staff ready to assist you and your loved one in meeting personal needs.

Each family member and resident is encouraged to be involved in interdisciplinary care conferences. Our interdisciplinary team is comprised of various professionals who are committed to working with you in the development of an individualized care plan for the resident.

Each resident is a unique individual with special needs and we are dedicated to a holistic approach, focusing on the physical, mental and psychological needs of our residents.
